Scarborough Centre for Healthy Communities - Community Health Centre and PAP Clinic

At SCHC we believe in a client-centered approach to primary health care. This means that our interdisciplinary team of health care professionals works with clients to ensure they are actively involved in the planning and decision-making about their care.

Note: Not a walk-in clinic, thus all visits require booking an appointment. All services provided are free of cost, we provide service to clients with OHIP, Interim Federal Health (IFH), and clients with no status. Services are not available to individuals in Canada on Visitors’ VISA, Student VISA or Super VISA.

Services Offered:

• Managing chronic health conditions such as hypertension, diabetes etc.
• Health Promotion (Preventing clients from becoming ill or injured)
• Treating acute (sudden onset) or episodic illness
• Referrals to/coordination with other health care professionals (such as hospitals and specialist care)
• Immunizations for children and adults
• Prenatal Care
• Counselling and Support Services

Medical Services:

• Periodic health exams
• Well-baby visits
• Sexual health screening services
• Smoking cessation

Other Services:

• Specializing in immigrant and refugee care
• LGBTQ2 safe space
• A physician specializing in developmental disabilities on site
